Complete all enemy entries in Tobin's Spirit Guide to 100%. Scan every Ghost with a 100% scan. There are 53 ghosts to scan in the game, like the artifacts, you can return to previous chapters and scan ghosts that were possibly missed. The easiest way to figure out if a Ghost was scanned 100%, is when Tolbin's Spirit Guide updates. Otherwise, the ghost will not appear in it.
